- Setting: Free Standing Clinic
- Dates of coverage needed: ASAP (quick credentialing) to July 1
- Schedule: Monday – Friday; 8a - 5p - will also cover at other locations
- Scope of Practice: Urgent Care, work related injuries, pre-employment physicals
- Experience: 2+ years of UC and Occ Med experience
- License and certification: Board Certified, OR State License, DEA, BLS, DOT Certification preferred
