• 01/01 7:00 AM – 01/04 7:00 AM

• 02/05 7:00 AM – 02/08 7:00 AM

• 04/02 7:00 AM – 04/05 7:00 AM

• 06/11 7:00 AM – 06/14 7:00 AM

• 07/09 7:00 AM – 07/12 7:00 AM

• 07/30 7:00 AM – 08/02 7:00 AM

Practice Details:

• Hospital setting

• 99-bed hospital

• EMR: Cerner

• 0–1 deliveries per shift

• Average 0–2 callbacks per shift

• Provider is expected to cover for the Nurse Midwife if there is a need

Qualifications:

• Board Certified or Board Eligible in OB/GYN

• Active AZ license or IMLC accepted

• DEA required

• Minimum 1 year of experience required
