📋 Requirements
✔ Board Certified OB/GYN (Board Eligible not accepted)
✔ Active Michigan Medical License
✔ Michigan CSR at time of submission
✔ BLS Certification
✔ DEA (verbal confirmation at name clear)
✔ NPDB Self-Query within the last 30 days
✔ Clean malpractice and background for quick credentialing
✔ Must be a local provider
