Qualifications:

• Board Certified in OB/GYN (No Board Eligible candidates).
• Active Michigan Medical License & CSR at submission.
• Current Basic Life Support (BLS) certification.
• Local provider required - must reside within close proximity.
• Clean background for swift credentialing.
• Must provide rates and availability when applying.
• Verbal confirmation of DEA required at name clear.
