- Schedule: Office coverage Monday to Friday, 8:00 AM - 4:30 PM, plus additional call
- Coverage Type: Scheduled Clinical Hours Plus Call

Requirements
- Board Certification: Eligible
- License Requirement: Licensed in KY only
