- Work Dates Needed: May 29th 7a - June 9th 7a
- Preferred Schedule: Clinic & Call
- - Clinic schedule - Monday is no clinic (Surgery Day), Tuesday 8:30-11:00AM in office, clinic on Wednesday 8:30- 4:30, Thursday 8:30-4:30, Friday 8:30- 11 AM. Will be the primary contact at night 5p – 8a.
- Worksite Setting: Inpatient/Outpatient
- Scope of Work: Clinic: On average about 25 patients on a full day. Approximately 12 patients/provider are seen on average, Support Staff 4-5 On-call average patient volume/consult 1-2 deliveries / week 100 deliveries/year.
- Licenses, Certifications, Requirements: Board Certified, IN State License, ABLS, ACLS, ATLS, BLS, PALS, DEA, Surgery Privileges
- EMR: Cerner
- Credentialing: 7-10 days
