Saint Alphonsus Medical Center in Ontario, Oregon is seeking a locum OBGYN provider for coverage starting in March!
- Dates Needed:
- March 15th at 0700 - March 18th at 0700
- March 22nd at 0700 March 23rd at 0700
- Schedule: Must be on site, no remote.
- 24 hour call coverage, beeper call
- Locums will NOT be responsible for supervising other providers
- EMR: EPIC
- Level II Trauma
- MD diploma
- Internship/residency/fellowship certs
- All medical licenses
- DEA
- Board Certification
- Life support cards- BLS
- Current and previous certificates of insurance
- Drivers license
- Professional color photo/headshot
- CMEs for the past 2 years
- Immunization records including TB, Flu, HepB, Tdap, Varicella, and COVID
