Background: The IHS, an agency within the Department of Health and Human Services, is responsible for providing federal healthcare services to American Indians and Alaska Natives. The provision of healthcare services to members of federally‐recognized tribes grew out of the special government‐to‐government relationship between the federal government and Indian tribes. The IHS is the principal federal health care provider and health advocate for American Indians and Alaska Natives, and its goal is to raise their health status to the highest possible level. The IHS provides a comprehensive health service delivery system for approximately 2.6 million American Indians and Alaska Natives who belong to 573 federally recognized tribes in 37 states.

Navajo Area Indian Health Service (NAIHS) administers health centers and hospitals providing health care to approximately 244,209 members of the Navajo Nation. The Navajo Nation is the largest Indian tribe in the United States and has the largest reservation, which encompasses more than 25,516 square miles in northern Arizona, western New Mexico, and southern Utah, with three satellite communities in central New Mexico. The NAIHS is the primary provider of inpatient, ambulatory care, preventive and community health, and environmental health services for members of the Navajo Nation and the San Juan Southern Paiute Tribe.

Chinle Comprehensive Health Care Facility (CCHCF) is a NAIHS facility. CCHCF provides medical care for approximately 35,000 Navajo. Serving a rural area, many of our patients travel 100 miles round trip to receive care and do not have electricity or running water in their homes. In addition, most of our elderly patients speak only Navajo, and live according to traditional Navajo cultural practices. The CCHCF and Ambulatory Care Center is a 60-bed inpatient hospital and outpatient facility. Services offered are:

Duties and responsibilities: shall encompass outpatient and inpatient, Hospitalist or Nocturnist Physician Services to IHS patients.

Place of Performance: Outpatient Hospitalist or Nocturnist Physician services shall be performed in the Emergency Room Department and Inpatient Hospitalist or Nocturnist Physician services shall be performed in the Inpatient Care Units: Adult Care Unit (ACU) & Special Care Unit (SCU); additionally, Internal Medicine/Pediatric Hospitalist or Nocturnist shall provide coverage in the Pediatric Care Unit (PCU) and the Nursery.

Period of Performance: Will be flexible to allow for short term (13 weeks), long term (1-year to 3-year) and, where feasible, include intermittent support to meet unplanned needs that develop on a frequent basis. Department requests for candidates to be available up to at least at minimum 4 weeks.

Work Schedule: Works shifts as arranged by the Primary Care Director or Chief of Inpatient Services or designee for this order.

Hospitalist Tour of Duty: 8:00 am to 8:00 pm, 12 hours per shift, 84 hours per week, Monday through Sunday, 7 days on and 7 days off.

Nocturnist Tour of Duty: 8:00 pm to 8:00 am, 12 hours per shift, 84 hours per week, Monday through Sunday, 7 days on and 7 days off.

There is no 24-hour shift on the weekend, contractor shall work 12 hours per shift both Saturday and Sunday as part of their seven days on. The contractor shall be subject to working longer/overtime hours if patient load demands are present to provide coverage in the Emergency Room and the Inpatient Units: Adult Care Unit, and Special Care Unit; Internal Medicine/Pediatric Hospitalist or Nocturnist shall provide coverage in the Pediatric Care Unit and the Nursery if necessary.

• Overtime Hours after 8:00 pm (Hospitalist) or 8:00 am (Nocturnist).
If needed, the contractor may be subject to work one Weeknight Call per week (possibly a Holiday).
• Weeknight On-Call Hours: 12 hours per night, 8:00 pm to 8:00 am, Monday through Friday.
• Holiday On-Call Hours: 12 hours per night, 8:00 pm to 8:00 am, Monday through Friday.

Experience/Training. The contractor shall have completed thirty-six (36) months of Family Practice or Internal Medicine Residency Training or have completed forty-eight (48) months of Internal Medicine & Pediatric Residency Training with a minimum of six (6) months of Hospitalist or Nocturnist experience required for this contract, unless otherwise approved by the CCHCF’s Credentials Committee. There must be no lapse of services of no more than 6 months prior to beginning tour of duty with CCHCF.

License/Registration. The contractor shall be board certified or board eligible with the American Board of Internal Medicine or American Board of Family Practice. Dual Internal Medicine-Pediatric providers shall be board certified or board eligible with the American Board of Internal Medicine and American Board of Pediatrics unless the individual will only be providing adult care. If the contractor is to perform out the normal training parameters of his/her specialty, then the contractor shall provide certification of hospital privileges in those additional scope of practice(s). The contractor shall maintain appropriate licensure, a valid, current, full and unrestricted license to practice as a Hospitalist or Nocturnist in any state within the United States, District of Columbia or the Common-Wealth of Puerto Rico, and certification throughout the term of this contract.

Shall maintain a valid and current Cardiopulmonary Resuscitation (CPR), Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) and are mandatory for Family Practice and Internal Medicine Hospitalist or Nocturnist. A dual Internal Medicine-Pediatric Hospitalist or Nocturnist shall also maintain a valid and current Pediatric Advanced Life Support (PALS) and Neonatal Resuscitation Program (NRP) certification.

Key Responsibilities

Direct Patient Care: Deliver professional medical services in alignment with standards of care established by recognized organizations and our own Medical Staff Bylaws and Rules & Regulations.

Comprehensive Patient Management: Conduct thorough patient evaluations, make diagnoses, and provide treatment or referrals as necessary. Engage in all aspects of patient care, from prevention and health maintenance to early diagnosis, treatment, and follow-up.

Critical Care and Specialty Services: Care for critically ill patients and, for dual Internal Medicine-Pediatric providers, offer services in the PCU and Nursery. Coordinate with specialists for comprehensive patient care.

Collaboration and Coordination: Work closely with a multidisciplinary team, including Community Health Nurses, Social Workers, and Health Educators, to manage patients' medical, psychological, and social needs.

Cultural Competence: Demonstrate a deep understanding of and respect for the cultural, social, and traditional contexts of our patient community. Participate in mandatory cultural competency training.
