Position Type: Locum Tenens

Location: South Bend, IN.
Shift: 24 Hour On Call
Start Date: May 28 (8a) and Dec 21 (8a) coverage blocks
End Date: June 1 (8a) and Dec 28 (8a)
Position Details:
Patient Population: Adult (100%)
Setting: Inpatient
Responsibilities: Inpatient neurology coverage, 24-hour call, patient evaluation and management
Required Procedures: Not specified
EMR System: Epic
Support Staff: 6 MDs in neuro between IP/OP, 1 APP, onsite lab and imaging
Days Per Month: Coverage blocks specified
On-Call/Weekends: 24-hour call including weekends
Submission Requirements:
- Board Certified
- Active IN License
- Conversion Fee acknowledgement ($25K cap)
- Self-Query NPDB report (within current month)
