- Practice Setting: Outpatient ALS office
- Dates of Coverage: Beginning 1/1/25
- What Percent of the Practice is Adult: 100%
- What Percent of the Practice is Outpatient: 100
- Average Number of Office/Clinic Patients Per Locum Per Day: 6-7
- Will Other Neurologists be Present During Locum's Assignment? No
- How Many Physicians in Your Group? 1
- Documentation System/EMR: EPIC
Facility will reimburse for the Following Costs: Hotel, Rental Car
Submission Requirements:
- Board Certified/Board Eligible: Palliative Care, Hospice, Neurology, Physiatry, even Internal Med or Critical care with experience in ALS
- NY License
- BLS/ACLS
