• Breakdown of Clinic Days/Sessions: Monday – Friday (2 sessions per day)
• Schedule of Days/Times Needed To Work/ Shift Hours: Monday to Friday 8 – 5 PM

Submission Requirements:

• • Board Certification: REQUIRED
  • Active NY License-REQUIRED
  • Fellowship: PREFERED
  • Certifications: BLS- REQUIRED
  • Ability to perform Botox, EMG:REQUIRED

Practice Description:

• Anticipated Procedures/Surgeries per Week Botox, EMG, EEG reads.
• Patient Load: Varies per day depending on number of new patient
• Is There Additional Surgical Staff? NO
• Is there additional support staff? MA, Admin staff, EMG tech
• Is the provider working solo or with other providers? If so, whom? Working with PA and NP
• Are there mid levels? If yes, is the locums provider responsible for mid levels? YES, Responsible for 1 NP
• Trauma Level of Work Location: LEVEL III
• EMR System? EPIC
