This practice operates 3 outpatient clinic locations and is contracted to staff 6 hospitals and 7 dialysis centers. All locations are within a 20-minute driving radius and providers are assigned to locations in 1-week blocks to offer continuity in schedule.

COMPANY PROFILE:
This multi-site nephrology practice has been serving patients in the Hudson Valley since 2012. They operate 3 outpatient offices of their own and provide nephrology services at 6 local hospitals and 7 local dialysis centers. They currently employ 8 physicians and 4 nurse practitioners, along with a robust support staff of administrative and clinical team members, and are hiring due to growth.

POSITION DESCRIPTION:
The new physician will rotate on a regular, pre-determined schedule between outpatient offices, hospitals, and dialysis centers. All locations are in a small geographic area (a 20-minute driving radius) and schedules are made in weekly blocks, so providers are consistently going to the same building or set of buildings for a week at a time and have a predictable, convenient schedule. The details of this are also flexible/negotiable.

The job duties and patient volume vary by work setting, but in all locations physicians are fully supported by clinical assistants, have all necessary resources, and use an EHR.

In the outpatient office:

- see 20 to 25 patients per day
- general outpatient nephrology care
- consultations
- follow-up visits
- PowerCharts EHR

In the hospitals:

- volume varies by hospital; largest is maximum of 35 to 40 patients per day
- rounding
- consultations
- NO admission duties
- serve as on-call provider (details below)

In the dialysis centers:

- minimal patient care
- oversight of nurse practitioners
- the patient care in the dialysis center is provided by NPs

The practice schedules 3 physicians to be on-call for the affiliated hospitals each weekend, with each provider assigned to a different area. One physician covers the northern territory, which includes 2 hospitals with a census of 25 patients. One physician covers the larger central territory with multiple facilities, working with a nurse practitioner who’s on-site for acute triage. One physician covers the southern smaller territory with a census of 15 to 25 patients. The rotation is currently 1 weekend every 3-4 weeks and there is minimal (if any) activity while on-call.

REQUIREMENTS:
- New York medical license
- DEA
- Board Certified or Eligible, Nephrology
- no experience required; current residents are encouraged to apply
- the practice is able to sponsor H1B visa candidates
