Requirements:

• Board Eligible (within 5 years of eligibility) or Board Certified in Neonatal-Perinatal Medicine - [REQUIRED]
• GA license (or willing to obtain) - [REQUIRED]
• Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
