Lead Physician for Post-Acute Care

  • Joining 3 other f/t Physicians and 1 Nurse Practitioners
  • Physician shall be an MD or a DO.
  • Physician shall be Board certified in Internal Medicine or Family Medicine. It is preferred that the physician be additionally certified in Geriatrics with experience in palliative care.

Duties as the Post-Acute Care Lead Physician

Physician’s duties shall be divided as follows:

  • Administrative oversight for the Post-Acute Care Practice
  • Performing duties as the Lead Physician
  • Group’s Post-Acute Care Practice.
  • Performing duties overseeing and collaborating with physicians and Advance Practice Providers in the Post-Acute Care Practice.
  • In a fiduciary capacity for group physician shall act as Medical Director of Post-Acute Care and Long Term Care Facilities.
  • Cultivate relationships and facilitate partnerships between group and Post-Acute Care/Long term care facilities, Assisted Living facilities and Acute Rehabilitation Facilities.
  • Conduct periodic performance evaluations on APPs and physicians in the Post-Acute Care Practice; at least annually.
  • Arrange for and participate in after-hours coverage of patients in nursing homes, Sub Acute Rehab Facilities, Long Term Care facilities and anywhere else that patients of the practice are located.
  • Communicate with facility (Post-Acute Care, Long Term Care, Assisted Living, Acute Rehab) staff and administrators
  • Communicate with the Divisional Directors of Primary Care, Hospitalist Service and Care Management
  • Create/build a process and infrastructure to facilitate safe transitions of care between inpatient Post-Acute Care, Outpatient and home.
  • Enhance referrals and engagement of post-acute patients with primary care for on-going care
  • Explore options to create/build infrastructure to provide group primary care services on site for Assisted Care and/or LTC patient
  • Provide input and advice regarding patient flow and accurate transmission of appropriate patient information to improve safe handoffs and transitions of care for patients both coming from and returning to the hospital and Post-Acute Care facilities.
  • Enhance palliative care, including POLST review, as appropriate for patients in post-acute settings
  • Reduce unnecessary hospital service from Post-Acute Care and Long Term Care facilities when appropriate care can be provided in the Post-Acute Care/Long Term Care facilities.
  • Enhance level of care provided in the Post-Acute Care/Long Term Care facilities.
  • Create clinical pathways specific medical conditions/disease states
  • Physician shall work at least 80 hours per pay period in clinical and administrative roles.

Lead Physician for Post-Acute Care Cont.….

  • Evaluate and treat patients in facilities. (Post-Acute Care, Long Term Care, Assisted Living, Acute Rehab)
  • Make decisions, order appropriate testing and solve problems; analyze data and evaluate results as indicated
  • Gather information: observe, receive and obtain information from all relevant sources.
  • Adhere to documentation standards.
  • Coordinate and improve communication amongst Hospital, Post-Acute Facility staff, Hospitalists, Primary Care Physicians, Emergency Department, and Care Management to enhance patient care quality across the continuum
  • Hold monthly practice meetings with Physicians, APCs, office manager and other relevant parties.
  • Meet quarterly with Facility Administrators along with Divisional Director – Primary Care
  • Meet Quarterly with Divisional Directors of Primary Care and Hospital Based Care to review professional performance of physicians, physician extenders and non-physician professionals providing clinical services within the Post-Acute Care Practice.
  • Collaborate with physicians and APPs on all admissions to partner facilities.
  • Develop and/or approve such clinical and operational protocols, procedures, rules and regulations as may be necessary.
  • Conduct regularly scheduled meetings with providers on system issues patient flow, quality, performance and improvement and promotion of professional development.
  • Assist with the establishment and enforcement of clinical, quality control programs, policies and procedures to ensure diagnostic and therapeutic reliability and safety of patients and personnel.
  • Utilize data from the Post-Acute Care program to enhance quality of the operation and strive for a high reliability program and culture.
  • The schedule will be based on the needs of the program and area covered.
  • Refer patients to medical specialists, social services, or to other professional as clinically appropriate.
  • Follow up on recommendations made by consultants as indicated.
  • Coordinate medical care, performing tasks to ensure that patients’ medical needs are met.
  • Write/approve patients’ discharge summaries and arrange for them to be sent to primary care physicians.
  • Participate in continuing education activities to maintain or enhance knowledge and skills.
  • Active participation in quality improvement initiatives. This can occur by involvement in hospital based committees, project leadership, or pilot programs that are intended to advance the care of patients.
  • Role in training and mentoring residents and allied health providers.
  • Adhere to the standards of faculty education when on resident services.
  • Collaborate and oversee the care provided by allied health providers. All admissions and discharges as appropriate shall be seen by the attending physicians, with appropriate co-signatures/documentation.
  • Attend documentation and billing compliance lectures as required by the department.
  • Attend and be prepared to present practice updates to quarterly meetings.
