Requirements:
• Active MA License
• BC in IM or FM & Geriatrics
• CDS & DEA
• Must have previous supervisory experience
• Most recent NPDB Report (within 30 days active)
• 1 year of documented experience working with a frail or elderly population
• Effective skills in physical assessment and chronic disease management for frail older adult
