Overview:
An established PACE (Program of All-Inclusive Care for the Elderly) organization in Fort Gratiot, Michigan is seeking a Medical Director to provide clinical leadership and direct patient care within a collaborative, interdisciplinary care model.
This is a leadership-focused role combining patient care with oversight of medical services, designed for physicians who value continuity, team-based care, and meaningful impact in a geriatric population.

• Permanent Medical Director opportunity
• Monday–Friday schedule (daytime hours)
• Hybrid flexibility (up to 2 days remote)
• Leadership + clinical balance
• Established patient panel (~150 participants)
• Team-based care model with strong support structure
• Fast-moving client with urgent need

• PACE program serving adults 55+
• Interdisciplinary care model (medical, social, behavioral)
• Supervising physician for 3 Nurse Practitioners
• Collaborative care planning and coordination
• Focus on quality outcomes over volume
This setting is designed for physicians who prefer depth of care and long-term patient relationships rather than high-volume throughput.

Responsibilities:
• Provide direct primary care to geriatric patients
• Oversee and guide clinical care across the program
• Supervise and collaborate with advanced practice providers
• Participate in interdisciplinary care planning
• Ensure compliance with clinical standards and regulatory requirements
• Build and maintain relationships within the local healthcare community

Requirements:
• MD/DO – Board Certified in Family Medicine or Internal Medicine
• Active Michigan license required
• DEA and BLS required
• Minimum 3 years post-residency experience
• Experience working with APPs or in team-based care models preferred
• Geriatric experience strongly preferred (fellowship or equivalent experience)
