We are seeking a Medical Director to lead our PACE (Program of All-Inclusive Care for the Elderly) model. This role focuses on providing comprehensive medical direction and oversight for seniors with chronic care needs to help them remain independent in their community.


-Schedule:
Monday – Friday; Full-time.
-Location: West Springfield, MA.
-Team: Provides direction and supervision to Primary Care Providers (PCPs) and Advanced Practice Providers (APPs); reports to the Executive Director.
-Charting: Administrative and clinical oversight within the PACE model.


Job Responsibilities

-Provide general medical direction and oversight of the Quality Assurance Performance Improvement (QAPI) program.
-Conduct history and physicals on new participants; develop medical treatment plans across various residential settings.
-Lead the interdisciplinary team (IDT) and participate in intake/assessment decisions.
-Manage medical policies, procedures, standards, and protocols.
-Educate participants and families regarding chronic medical conditions and health maintenance.

Qualifications

-Board Certification: Board Certified in Internal Medicine or Family Practice [REQUIRED]. Geriatrics certification [PREFERRED].
-Licensure: Current and active plenary physician license in Massachusetts [REQUIRED].
-Credentialing: Self-Query NPDB report required at time of presentation (pulled within the same month).
-Experience: Minimum of one (1) year of documented experience working with a frail or elderly population [REQUIRED]. Previous supervisory experience [REQUIRED].

Working Hours

-Shifts: Standard business hours (M-F).
-On-Call: Participation in provider on-call rotation as required.
-Availability: Locum placement; full-time

Skills

-Proficiency in physical assessment and chronic disease management for frail older adults.
-Strong leadership and collaborative skills for an interdisciplinary environment.
-Ability to manage changing priorities in a creative alternate healthcare setting.
