Permanent Medical Director – Geriatric Medicine
Position Type: Permanent
Specialty: Physician – Geriatric Medicine
Location: Indiana.
Shift: Full time, day shift
- Patient Population: Frail/Elderly (PACE participants 55+)
- Setting: PACE (Program of All-Inclusive Care for the Elderly) outpatient/center-based care
- Responsibilities: Provide medical direction and supervision of participant care, oversee clinical outcomes and QAPI, supervise PCPs and APPs, conduct H&Ps and reassessments, develop treatment plans, lead interdisciplinary team collaboration, participate in intake/assessments and family conferences
- Required Procedures: Physical examinations, chronic disease management, ordering/evaluating labs and diagnostics
- EMR System:
- Support Staff: Interdisciplinary team including PCPs, APPs, nurses, therapists, social workers
- Days Per Month: Full time
- On-Call/Weekends: No call, No weekends
Submission Requirements:
- Board Certified in Internal Medicine or Family Medicine
- Board Certified or Fellowship/CAQ in Geriatric Medicine
- Active Indiana license (or willing to obtain)
- NPDB Self Query within 30 days
- Minimum 1 year experience with frail/elderly population
- Supervisory experience
- DEA, CDS, BLS (by start)
