Medical Director or Associate Medical Director (FP, IM, Geriatrics) – Post-Acute & Primary Care - Albany, NY – New York’s Capital Region

Join a leading healthcare organization delivering high-quality post-acute and primary care services across the Albany area. We are hiring for two leadership-track positions based on your experience and interest: Medical Director or Associate Medical Director.

Opportunity Highlights:

• Full-time roles (4–5 days/week) in skilled nursing facility settings
• Weekday schedule – no nights or weekends
• Focus on quality outcomes, care coordination, and clinical oversight
• Provide direct patient care and support interdisciplinary teams
• Participate in leadership meetings, quality initiatives, and case reviews

Position Tracks:

Medical Director – Post-Acute Care

• Lead a team of clinicians across multiple SNF locations
• Drive initiatives in readmission reduction, patient-centered care, and outcomes improvement
• Collaborate with pharmacists, facility administrators, and nursing leaders
• Requires prior Medical Director experience in SNF or post-acute setting

Associate Medical Director – Primary Care

• Provide hands-on clinical care while mentoring nurse practitioners
• Oversee care quality and participate in provider development
• Ideal for physicians with leadership interest and SNF/long-term care experience

Candidate Requirements:

• Board Certification in Internal Medicine, Family Medicine, or related specialty
• Active New York medical license and DEA
• Minimum 2 years of experience in post-acute, SNF, or similar care environment
• Leadership experience required for Medical Director role
• Strong communication and collaboration skills

Compensation & Benefits:

• Medical Director: Estimated $310,000–$345,000/year (uncapped fee-for-service)
• Associate Medical Director: Estimated $273,000–$318,000/year (uncapped productivity)
