- Monday - Friday Clinic Coverage on site: 9 Hours
- On call coverage TBD
- Epic and Viewpoint used
- Consults and US reads
- Minimum of 1 week block coverage a month
Requirements:
- Board Certified in MFM [REQUIRED]
- WA Active License or IMLC Eligible for Compact License [REQUIRED]
- Self- Query NPDB pulled out within 30 days of submission. [REQUIRED]
- Cannot accept providers who have not been clinically active for 1 year [REQUIRED]
- Providers must have at least 1 year post fellowship experience [REQUIRED]
- Must have completed Prescreen Questionnaire (see attachment for form) [REQUIRED]
- CV must be in MM/YYYY format with all gaps explained [REQUIRED]
- Rates must follow rate structure [REQUIRED]
- Ideal availability is a min of a 1 week block per month [REQUIRED]
- No malpractice or disclosure history [REQUIRED]
- Home Address, Phone Number, and Email address to be added in CV's first page [REQUIRED]
