Collegial and Integrated team seeks a Family Medicine Physician or Internal Medicine Physician or Palliative Care or Geriatrician

- low turnover group with their MDs and NPs
- team setting / collocative, not a solo position. Its a team of OT/PTs, NPs, PAs, psychologist, social workers
- primary care care 40 hrs a week, 3-10 patient per day
- 200-240k yr plus 5k yr in call. call 1 week every 8 weeks 1:8
70% dementia patients, 370 patients in the program now. 4 year life expectancy in the program
- 3 days in Muskegon and 2 in Holland
- goal is to keep care in house versus always referring to specialists
