Job Summary:_x000D_
Group is actively seeking a board-certified physician to join our group, providing high quality primary care in an office-based setting. _x000D_
_x000D_
Job Responsibilities:_x000D_
Examine, obtain medical history and treat acute and chronic illnesses._x000D_
Order, perform and interpret diagnostic tests._x000D_
Create treatment plans, and involve patients and family members in those plans._x000D_
Initiate appropriate specialty referrals._x000D_
Collaborate on improvement in care delivery and service with management, other physicians and staff._x000D_
Work with care team to close care gaps and improve quality measures._x000D_
Participate in Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives. _x000D_
Work with practice management to ensure compliance with billing and coding requirements._x000D_
Provide on call coverage within the primary care group._x000D_

Education/Licensing Requirements:_x000D_
Graduation from accredited medical school (MD or DO)._x000D_
Graduation from accredited Internal Medicine or Family Medicine residency program._x000D_
Valid and unrestricted Connecticut medical license or ability to obtain._x000D_
Valid DEA registration and Controlled Substance Certificate._x000D_
Board certified in Internal Medicine or Family Medicine._x000D_
