Geriatric PACE Program- Position Details

*PACE Program*

-Patients: 8-10 patients per day, 60-90 minute visits, all geriatric

-Scope: 100% outpatient geriatric

To ensure continuity, quality of care, each day is started with a meeting between our interdisciplinary staff that includes dentists, social workers, psychiatrists, mid-levels, and other primary care providers. We discuss recent patient visits, plans for those patients' care, and plan our day accordingly.

-Required: Board Certified or Board eligible

-Malpractice - PAID for locum opportunities.

-Duration: December 2019 – June 2020 (per your availability)

-Location: Philadelphia, PA

-Schedule: Monday – Thursday, 8a-5p and Fridays 8a-1p (with one-hour lunch each day)

-Scope: Outpatient, adult, primary care

-Patient volume: 18 patients per day (once ramped up)

-Affiliated Hospital: 700 bed teaching facility

-Required: Board certified or board eligible in Internal Medicine with Pennsylvania license

Location Chicago- Position Details:

-Duration: Now open – on-going (will consider locum tenens to permanent after 90 days)

-Location: 3 locations in suburban Chicago (zip codes: 60625, 60647, 60085)

>must be within driving distance, hotels/travel are not offered

-Schedule: Monday – Friday, 8a-5p

>option for extended 7:30 hours 2 nights per week and/or 2 Saturdays per month

-Scope: Outpatient, primary care

-Patient volume: 15-18 per day (once ramped up)

-Patient age range per your comfort level

-Group: 2 family medicine physicians, 4 pediatricians, 3 OBGYNs, 7 Advance practice providers

-Required: Board certified or board eligible with Illinois license
