Join a Community Health Center with 150+ Providers (45+ Physicians and 100+ APP’s) in 25+ clinics and 35+ LTAC facilities as they grow.
They are expanding their Post Acute Care / Long Term Care /Skilled Nursing Facilities team.
Primary responsibilities include overseeing patient care at area nursing homes in post-acute care, long term care and assisted living.
- Facilities are established, long term and short term
- 100 beds
- Cover ONE facility
- ~ 3 patients / hour
- NP's on staff and NP's do discharges
- PM&R & Behavioral Health physicians on site.
- Monday - Friday
- no nights
- no weekends
- 8 AM – 5 PM
- Dragon dictation with AI scribe
- Skilled Nursing Facilities / Long term care facilities
- EMR: eClinicalWorks
- Part-time positions available
Benefits:
- Base salary - 320k - 350k DOE
- Sign on - 20k
