Job Responsibilities
• Manage chronic conditions and preventive health services
• Coordinate care across specialists, hospitals, and post-acute services
• Participate in team-based care model and daily clinical huddles
• Ensure accurate and timely documentation with support staff
• Contribute to quality metrics and patient outcome improvements
Qualifications
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active or eligible Mississippi medical license
• DEA license required
• Strong interpersonal and patient care skills
• Experience in value-based or Medicare Advantage models preferred
