We are seeking a compassionate and driven physician to join our post-acute care team in the Rochester, New York area. This full-time position (5 days/week) involves rounding in a skilled nursing facility and offers strong clinical support, autonomy, and excellent earning potential.

Compensation & Incentives
  • Estimated base salary: $243,488 – $288,186 annually
  • Uncapped productivity-based income potential

What You’ll Do
  • Conduct comprehensive patient assessments, including medical histories and physical exams
  • Develop and manage individualized care plans
  • Monitor patient progress and coordinate care for optimal outcomes
  • Collaborate with physicians, nursing staff, and interdisciplinary teams
  • Educate patients and families on health management and preventive care
  • Maintain accurate and compliant medical documentation

Qualifications
  • Active physician license in the state of New York and DEA
  • Experience in post-acute, acute care, emergency department, or clinic settings preferred (new graduates encouraged to apply)
  • Strong clinical knowledge and understanding of healthcare regulations
  • Excellent communication and interpersonal skills
  • Collaborative and team-oriented mindset
