Facility in Cambridge Massachusetts is searching for a Family Medicine, Internal Medicine or Geriatrics Physician
- Open to Physicians Only
- Provide comprehensive primary care to PACE participants
- Manage chronic and acute medical conditions
- Participate in interdisciplinary team meetings
- Coordinate care with nursing, social work, and therapy staff
- Document care in the electronic medical record
- Smaller patient panels due to high patient complexity
- Work at a Long Term Care Facility
SUPPORT STAFF
- APP and Physician coverage: Yes
- EMR: Epic and PointClickCare. Epic training and coding training required
MAX BILL RATES: $214 (VMS fee taken out)
ACTIVE MA LICENSE REQUIRED
FLU VACCINATION REQUIRED - NO EXCEPTIONS/WAIVERS NOT ACCEPTED
Patient Volume: Six to eight patients per day including new admissions and follow up visits
Care Model:
On site Nurse Practitioner support at each facility
NPs manage nursing calls orders and medication refills
NPs complete initial admission visit within seventy two hours
Physician completes new admission visit typically within seven days
Long term care patients require physician visits every sixty to ninety days unless acute needs arise
