Primary Care Facility Medical Director – Post-Acute Care
Oxford, New York | Full-Time (4 Days/Week)
We are seeking a Primary Care Facility Medical Director to provide high-quality patient care in a post-acute setting within skilled nursing facilities in Oxford, New York. This leadership opportunity allows you to combine clinical practice with mentorship and program leadership while maintaining schedule flexibility.
In this role, you will round four days per week with the flexibility to choose your rounding schedule while supporting patients and clinicians in the skilled nursing facility setting.
Job Responsibilities
- Provide primary care rounding in skilled nursing facilities (SNFs)
- Manage chronic diseases, diagnose illnesses, and develop individualized plans of care
- Collaborate with clinicians, facility leaders, and interdisciplinary teams to coordinate patient care
- Build meaningful relationships with patients, families, and facility staff to support effective care delivery
- Provide oversight, guidance, and mentorship to nurse practitioners
- Conduct clinical case reviews and share best practices related to quality of care
- Partner with leadership to review care coordination initiatives and develop collaborative intervention strategies
- Participate in discussions related to network management and care delivery improvements
- Minimum of 1 year of primary care experience
- Post-acute care experience preferred
- Active and unrestricted New York physician license (or application in process)
- Active DEA/prescriptive authority (or application in process)
- Estimated annual compensation: $231,800 – $246,651
- No cap on productivity income potential
