Location: Augusta, GA
Employment Type: Full-Time, Permanent

Responsibilities:
- Provide comprehensive primary care including preventive services, screenings, and lifestyle counseling
- Diagnose and manage acute and chronic conditions such as diabetes, hypertension, COPD, and heart disease
- Conduct mental health screenings and coordinate referrals as needed
- Lead care coordination with specialists, mental health providers, and allied health professionals
- Educate patients on health promotion, disease prevention, and healthy lifestyle practices
- Perform telehealth visits, including video and telephone consultations
- Maintain accurate documentation and compliance within the electronic health record
- Review and coordinate patient admissions, referrals, and follow-up care

Qualifications:
- Board Certified in Internal Medicine (MD or DO)
- Completion of an accredited Internal Medicine residency
- Active physician license in any U.S. state with DEA and NPI
- Current ACLS and BLS certifications
- Commitment to patient-centered primary care
- Experience in team-based clinical settings and telehealth preferred
