General Primary Care, chronic disease management, preventive care, mental health screening, women's health, geriatric care, palliative care
Chronic disease management (Diabetes, Hypertension, COPD, Heart Failure), acute care evaluations, mental health screening (PTSD, Depression, Substance Use), preventive care, geriatric & palliative care

Job Responsibilities
New and follow-up patients
90% time in direct patient care; responsible for test result communication, referrals, and care coordination
EMR: Oracle/Cerner
Collaboration with PACT team: Social Workers, Pharmacists, Community Care providers, Mental Health specialists
Qualifications
Board Certified/Board Eligible
Working Hours
Monday–Friday, 8:00AM–4:30PM
Approx. 40 hours/week clinic + on-call as assigned
