We are currently seeking Family Medicine, Internal Medicine, or Geriatrics providers to join a PACE (Program of All-Inclusive Care for the Elderly) setting in Cambridge, MA. This opportunity is open to Physicians, Nurse Practitioners, and Physician Assistants who are passionate about caring for complex older adults and medically fragile patients.

The PACE model focuses on coordinated, patient-centered care, allowing providers to work closely with interdisciplinary teams to improve the health and quality of life of elderly participants.

πŸ”Ή Provide comprehensive primary care for PACE program participants

πŸ”Ή Conduct clinic visits, home visits, and nursing home visits as assigned

πŸ”Ή Coordinate care with nursing, social workers, therapists, and support staff
πŸ”Ή Manage patient communications such as phone and in-basket coverage
πŸ”Ή Document patient encounters using Epic EMR
