The Syracuse Veterans Affairs Medical Center is seeking a Board Certified Primary Care Physician to provide care to home bound veterans.

The Physician provides patient care directly or through the supervision of residents in either the home or outpatient settings or in collaboration with LIPs. In addition to the role as clinician, the Physician may have administrative responsibilities for leadership and direction of programs in HPBC , as well as effective interface with the Ambulatory Care staff. The Physician may participate in accreditation processes, reviews, and site visits. Duties include but are not limited to:

  • Assuming primary medical responsibility for HBPC patients and conducting clinical examinations in the patients' homes
  • Identifying the patients' medical problems, and the medical management of these problems
  • Determining the need for consultation from medical, surgical, and/or psychiatric subspecialty clinics
  • Determining the need for, and facilitating admission to the hospital
  • Serving as collaborator and/or Physician delegate for advanced practice nurses
  • Providing medical administrative support such as signing forms requiring Physician signature for the HBPC patients
  • Serving as acting Medical Director upon the absence of the Medical Director
  • Plan and direct the educational and clinical experience of medical students, residents, and fellows assigned to HBPC
  • Assume a leadership role in the development and implementation of the HBPC performance improvement plan
  • Advocate for HBPC with VHA leadership and the medical community
  • Providing clinical input and oversight for patient treatment plans for HBPC
  • Participating in HBPC team meetings, keeping the HBPC teams apprised of medical care advances and practice standards
  • Arranging Physician coverage and communicating the plan of coverage to the HBPC team
  • Collaborating with other HBPC Medical Directors, VISN leadership, and VA Central Office staff on program development issues
  • Collaborating with community home health and hospice agencies as needed for HBPC patients
  • Following HBPC Staff Safety procedures
  • Complying with Medical Center and VHA regulations regarding the use of government vehicles
  • Demonstrating work practices that include adherence to Infection Control standards and the safe use and operation of equipment

Work Schedule: Full time, Monday-Friday 8:00am - 4:30pm (or as determined by the needs of the service)
