Position OverviewWe are seeking Board Certified or Board Eligible Physicians in Internal Medicine or Family Medicine to join our expanding House Calls Division. This role provides comprehensive primary care to patients in their homes, focusing on individuals with limited mobility or access to traditional healthcare settings.
Physicians in this role must demonstrate strong clinical skills, compassion, and independent decision-making while coordinating care with interdisciplinary teams. After a successful probationary period, this role may include supervisory responsibilities as a medical director.
Key ResponsibilitiesPatient Care & Assessments

Conduct comprehensive evaluations of patients’ medical histories, physical condition, and medications during home visits

Diagnose and develop individualized treatment plans, including prescribing medications

Address urgent medical needs during home visits and provide appropriate referrals
Coordination & Collaboration

Collaborate with mid-level providers, nurses, specialists, and community health workers to ensure continuity of care

Assist in arranging additional patient services such as lab tests, physical therapy, or home health nursing
Team Leadership & Mentorship

Supervise and guide interdisciplinary teams, providing clinical mentorship and teaching

Support administrative oversight and quality improvement initiatives
Patient Education & Documentation

Educate patients and families on health management, medication adherence, and lifestyle modifications

Maintain accurate and up-to-date medical records for all visits, including assessments, treatments, and follow-up plans
Travel

Conduct home visits across the designated service area, managing an organized schedule

MD or DO from an accredited medical school

Successful completion of a residency in Internal Medicine or Family Medicine

Board Certification or Eligibility in Internal Medicine or Family Medicine

Active New York State Medical License and DEA registration

Minimum of 5 years clinical experience post-training

Experience in primary care, diagnostics, and collaborative team-based care

Geriatrics and/or palliative care experience preferred but not required

Full-time applicants preferred

Annual salary: $280,000 – $300,000 (base + performance incentives)
