Assignment Highlights

• Location: Middlebury, VT

• Start Date: ASAP

• Practice Setting: Outpatient | Long-Term Care (98-bed facility)

• Patient Volume: 8–12 patients per day

• EMR: Epic

Schedule That Works

• Monday–Friday | 8:00 AM – 5:00 PM

• 8-hour shifts

• No nights, no weekends, no call

Clinical Support

• 1 highly experienced full-time APP on site

• Additional APP support (.5 FTE) in progress

• Built-in rotating physician coverage

• Strong nursing staff, leadership, and interdisciplinary support

Compensation

• Competitive rates - to be discussed upon interest

Requirements:

• Board Certified or Board Eligible in Internal Medicine or Family Medicine

• Active DEA

• Vermont license or IMLC accepted

• Comfortable practicing in a long-term care setting
