Primary Care Physician Opportunity
Location: Rutland, VT - Manchester, NH 120m; Albany, NY 90m
Setting: Multi-site Community Health / FQHC
Employment Type: Full-Time, Employed
Schedule: 4-day work week with flexible hours
Position Overview
* Outpatient primary care role serving a diverse patient population
* Practice within a multi-site primary care system across central Vermont
* Opportunity to provide comprehensive, longitudinal care focused on prevention and wellness
* Minimal after-hours call shared across sites (triage only)
* Supportive team-based care model with strong clinical infrastructure
Clinical Responsibilities
* Provide comprehensive primary care services to patients of all ages
* Promote preventive care, wellness, and chronic disease management
* Empower patients to actively participate in their health care
* Provide appropriate follow-up and clear patient communication
* Refer patients to specialty care as clinically indicated
* Provide clinical supervision to advanced practice providers and nursing staff
* Participate in quality assurance and quality improvement initiatives
* Engage in state and national disease collaborative efforts

Qualifications
* MD or DO
* Board certification strongly preferred
* Eligible for licensure in the state of Vermont
* Completion of accredited medical education and residency training
