Internal Medicine Physician – Post-Acute Care

We are seeking a full-time Internal Medicine physician (MD or DO) to join our team and provide high-quality care in post-acute care settings throughout Worcester, MA. This position offers a manageable patient census, strong interdisciplinary support, and uncapped earning potential.
Position Overview
The physician will be responsible for delivering safe, effective, and cost-conscious care to patients in post-acute care communities. This role includes inpatient rounding across multiple care settings and close collaboration with interdisciplinary teams to ensure optimal outcomes and smooth care transitions.
- Patient volume: 15–20 patients per day
- Employment type: Full-time
- Compensation model: Fee-for-service (FFS)
- Estimated annual compensation: $252,000 – $302,000, with no cap on productivity income
- Benefits eligible full-time role
- Provide high-quality, patient-centered care in post-acute care communities
- Perform inpatient rounding in skilled nursing facilities, inpatient rehabilitation facilities, and long-term care hospitals
- Collaborate with interdisciplinary clinical teams while adhering to CMS and Medicare guidelines
- Coordinate care transitions with hospice, palliative care, and home care agencies
- Ensure timely, accurate documentation and billing using the Gehrimed EMR system
- MD or DO
- Board certified in Internal Medicine or Family Medicine
- Active medical license in the state
- Strong clinical judgment and commitment to quality patient care
