What You’ll Do

Provide comprehensive outpatient primary care in a value-based care model

Care for an established 65+ patient population

Collaborate within a multidisciplinary, team-based environment

No inpatient responsibilities
Practice Highlights

100% outpatient, Monday–Friday schedule
-
One work-from-home day per week

Approximately 18 patients per day

Limited call responsibilities

Robust team-based support including EMTs, APPs, Medical Assistants, Care Managers, and acute care nursing support
