Post-Acute Care Physician (FP, IM, Geriatrics) – Delaware County, PA (Philadelphia Suburb)
Opportunity Highlights:
- Full-time (5 days/week) SNF-based position
- Focus on primary care, chronic disease management, and quality outcomes

- Participate in care coordination, quality initiatives, and case reviews
Compensation & Benefits:
- Estimated annual income $180,000–$291,000 (FFS, uncapped)

Candidate Requirements:
- BC/BE in IM, FM, EM, or related specialty
- Active or in-process PA license and DEA
- Minimum 1 year of relevant experience preferred
