Setting: 100% Outpatient | No Nights | No Weekends | No Call
Patient Population: Primarily Geriatric / Medicare
Compensation: Competitive base salary + bonus potential + full benefits
An established outpatient primary care clinic on the South Side of Chicago is seeking a Board-Certified or Board-Eligible Internal Medicine or Family Medicine Physician to care for a predominantly geriatric, Medicare-focused patient population.

Role Overview

Provide comprehensive outpatient primary care to older adults

Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and arthritis

Emphasize preventive care, care coordination, and patient education

Collaborate closely with nursing staff, medical assistants, and care coordinators

Practice within a value-based care model focused on outcomes, not volume
Qualifications

MD or DO
-
Board-Certified or Board-Eligible in Internal Medicine or Family Medicine

Active Illinois medical license or ability to obtain

Interest in outpatient, geriatric-focused primary care
