Primary Care Physician | PACE Program

Join a mission-driven, value-based care team as a Primary Care Physician in our PACE (Program of All-Inclusive Care for the Elderly) center located in Stockton, CA. We are seeking full time physicians who are passionate about delivering holistic, patient-centered care to older adults through an integrated care model that supports clinical autonomy and meaningful relationships with patients.

As a Primary Care Physician in our PACE program, you will provide comprehensive primary care to frail seniors, collaborating with an interdisciplinary team to ensure optimal health outcomes. This role includes clinical leadership and offers a unique opportunity to make a lasting impact in a supportive, team-based environment.

Position Highlights:

• Schedule: Monday–Friday, 8:00 AM – 5:00 PM; no weekends
• Includes occasional home visits and rotating on-call coverage (phone only, a few nights/weekend days per month)
• Patient Load: Clinic: 8 patients per day, Field: 4 patients per day

Compensation & Benefits:

• Sign-on Bonus $50,000
• Competitive Compensation: $315,900 – $386,600 base salary, plus quarterly bonus

Requirements:

• Board Certification in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine
• Be eligible to enroll with Medicare and Medi-Cal
• Meet requirements for COVID-19 vaccination or provide a declination form prior to employment
