Family Medicine or Internal Medicine – Post Acute Care – Boca Raton, Florida

Position Highlights:

• Full-time position (4–5 days per week)
• Inpatient rounding in skilled nursing facilities, inpatient rehab, and long-term care
• Average of 15–20 patients per day
• No primary night call
• Collaborate with interdisciplinary clinical teams and support smooth care transitions
• EMR: gEHRimed

Compensation & Benefits:

• Productivity-based compensation (Fee-for-Service); estimated range: $235,000–$278,000 annually
• No cap on monthly productivity income
• W2 employed position with full benefits

Candidate Requirements:

• Board-certified in Family Medicine, Internal Medicine, or Emergency Medicine
• Active Florida medical license (or eligibility)
• DEA license (or ability to obtain)
• Prior post-acute care experience preferred
• Minimum of 1 year primary care experience
