We are seeking a dedicated Locums Internal Medicine Physician to provide high-quality outpatient care across multiple Long-Term Care (LTC) and Assisted Living (ALF) facilities in Hickory, NC, Roanoke Rapids, NC and Columbia, SC area's. This role focuses on delivering compassionate, patient-centered care to an aging population while supporting facility staff, patients, and families throughout the post-acute continuum.

Position Details

  • Setting: Outpatient care in multiple Long-Term Care and Assisted Living facilities
  • Location: Physicians will cover multiple
  • Full and part time options available
  • Schedule: Monday–Friday, 8:00 AM – 5:00 PM
  • Daily Patient Volume: 18+ patient visits per day
  • Call: Possible night call (frequency TBD)

Responsibilities

  • Provide comprehensive care to residents in LTC and ALF settings.
  • Manage chronic medical conditions and acute issues for geriatric and medically complex patients.
  • Conduct daily patient visits, ensuring at least 18 encounters per day.
  • Collaborate with nursing staff, therapists, and facility administrators to ensure coordinated care.
  • Offer clinical expertise, communication, and support to patients and their families.
  • Serve as an advocate and resource as patients transition through:
    • Acute Inpatient Rehab
    • Skilled Nursing
    • Long-Term Care
    • Assisted Living
    • Transition back to home
  • Maintain thorough documentation and follow facility protocols.
  • Participate in night call rotation when assigned.

Requirements

  • Board Certified or Board Eligible in either:
    • Internal Medicine
    • Family Medicine
    • Geriatrics
  • Active North Carolina and South Carolina medical license
  • Experience in post-acute, geriatric, or facility-based care preferred
  • Strong communication skills and ability to work well in interdisciplinary environments
  • Compassionate approach to complex and aging populations
