Job description Director of Health Information & Population Health – Santa Cruz County, CA

Position Type: Full-time, Outpatient + Administrative EMR: Greenway Intergy Spanish: Strongly preferred

Role Overview

A mission-driven community health center in Santa Cruz County is seeking a Director of Health Information & Population Health. This leader will guide data, quality, and informatics initiatives for an 11-site system while maintaining a small outpatient clinical panel.

Key Requirements
  • Strong experience with EHR implementation or transition

  • Clinical informatics background or certifications highly valued

  • Spanish-speaking strongly preferred

  • No prior director-level leadership required—open to strong emerging leaders

  • Cannot sponsor visas

Compensation
  • Base salary: $270,400 – $304,337.58 (DOE & FTE)

  • Sign-on/Retention Bonus: Up to $30,000

    • 50% at start, 25% after year 1, 25% after year 2

  • Relocation: Up to $10,000

  • Benefits:
