Role OverviewA mission-driven community health center in Santa Cruz County is seeking a Director of Health Information & Population Health. This leader will guide data, quality, and informatics initiatives for an 11-site system while maintaining a small outpatient clinical panel.
Key Requirements-
Strong experience with EHR implementation or transition

Clinical informatics background or certifications highly valued

Spanish-speaking strongly preferred

No prior director-level leadership required—open to strong emerging leaders

Base salary: $270,400 – $304,337.58 (DOE & FTE)

Sign-on/Retention Bonus: Up to $30,000

50% at start, 25% after year 1, 25% after year 2
