Primary Care Associate Medical Director | Albany, New York
Position Type: Full-Time | 4–5 Days per Week | Flexible Rounding Schedule
Compensation: $273,488 – $318,185 annually (no cap on productivity income potential)
About the Opportunity:
Join post-acute care leadership team in Albany, New York, as a Primary Care Associate Medical Director. This full-time role offers an excellent balance of clinical care and administrative leadership while providing flexibility and autonomy in your schedule. You will oversee quality patient care and mentor clinicians across skilled nursing facilities (SNFs) in the region.
Responsibilities:
Provide primary care rounding for adult patients in skilled nursing facilities
Manage chronic conditions, diagnose illnesses, and implement treatment plans
Collaborate with facility leaders, clinicians, and multidisciplinary teams
Offer oversight, mentorship, and clinical guidance to nurse practitioners
Conduct clinical case reviews and share best practices to enhance quality of care
Represent the organization within the post-acute care community and develop collaborative relationships
Support leadership initiatives around care coordination and quality improvement
Qualifications:
MD or DO, Board Certified/Board Eligible in Family Medicine or Internal Medicine
Minimum of 1 year of primary care experience; post-acute care experience preferred
Active and unrestricted New York medical license and DEA (or in process)
Strong interpersonal and leadership skills with a patient-centered approach
Compensation & Benefits:
