About the Position:
Value-based care offering extended time with patients and the ability to build trusting, long-term relationships
Focus on quality and making a difference for your patients - not wRVUs
30 minute – 1 hour appointment slots for all patients - average 10-14 patients per day
Managing a patient panel size of 600 or 1,000 with APP support
Dedicated panel RN for in-basket messages, telephone encounters, triage, and support
Microsoft DAX and other technology tools to significantly improve efficiency and work/life balance
In-person, virtual, or telephonic encounters – annual comprehensive visits, transitional care, and acute/urgent visits
Exceptional work/life balance with administration dedicated to removing barriers
Administration dedicated to reducing physician burnout with robust physician wellness plan and wellness committees
No call, holidays, weekends, or evenings lead to great work/life balance
