#### Job Responsibilities:
Provide comprehensive primary care rounding in skilled nursing facilities (SNFs)
Manage chronic illnesses and acute medical conditions
Develop and implement personalized plans of care for each patient
Collaborate closely with clinicians, nursing staff, and facility administrators
Build and maintain relationships with patients and their families
Provide oversight, guidance, and mentorship to nurse practitioners
Conduct clinical case reviews and contribute to quality-of-care initiatives

#### Qualifications:
Board Certified/Board Eligible in Internal Medicine, Family Medicine, or related specialty
Minimum of 1 year of primary care experience required
Post-acute care experience preferred but not required
Active and unrestricted New York medical license (or ability to obtain)
Active DEA or eligibility to apply

Estimated Annual Compensation: $183,222 – $221,114
