Utica, New York, Post-Acute Care Physician Opportunity
Clinical Role:

- This full-time primary care physician role will include patient care rounding in a post-acute care setting in a skilled nursing facility (SNF) located in the Utica, NY, area. You will round 5 days a week with the flexibility to choose your rounding schedule.
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Collaborate with clinicians, facility leaders and others regarding care and services
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Provide oversight, guidance, and mentorship to our nurse practitioners
- Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care
- Minimum of 1-year primary care experience
- Post-acute care experience a plus, but not require
- Hospitalists, internal medicine, family medicine, emergency medicine and related specialties are encouraged to apply
- Active and unrestricted New York license or application in process
- Active and unrestricted DEA/prescriptive authority or application in process
- Expected compensation is $183,222 to $221,114 annually with no cap on productivity income potential.
