Position Summary
Under the supervision of a Medical or Specialty Director, the Provider delivers culturally competent, high-quality care in a community-based, team-oriented setting. This role encompasses the diagnosis and treatment of physical and behavioral health conditions, preventive health maintenance, patient education, and continuity of care for Unity patients.
Minimum Qualifications
- Current DC professional license (MD/DO, NP, or PA)
- Active CDS and DEA registrations (DC-based)
- Graduate of an accredited medical, nursing, or PA program
- Board certified or board eligible in specialty area (must be certified within 1 year of hire)
- Willingness to participate in a rotating on-call schedule and occasional weekend/evening coverage
Work Environment & Risks
The role involves regular clinical work within Unity Health Care facilities. Standard safety precautions apply. Providers are expected to comply with medical community standards and provide documentation of a recent health assessment (including PPD or chest X-ray results).

Job Responsibilities
Health Outcomes
Perform comprehensive clinical evaluations, diagnostic testing, and develop treatment plans
Provide preventive care, patient counseling, and specialty referrals as needed
Participate in quality improvement initiatives and peer reviews
Maintain timely and accurate documentation in the electronic medical record
Stay current with clinical best practices through continued medical education (CME)
