An outstanding full-time opportunity for a Board-Certified Family Medicine or Internal Medicine Physician with Geriatric Medicine Fellowship training or CAQ in Geriatrics to serve as Medical Director of a Program of All-Inclusive Care for the Elderly (PACE) in Mishawaka, Indiana.

This leadership role oversees the medical direction of the program, ensures high-quality care delivery, supervises clinical providers, and drives quality outcomes for a frail elderly population.

Job Responsibilities

Provide medical direction & oversight of participant care.

Supervise Primary Care Providers (PCPs) & APPs.

Develop & maintain medical policies, procedures, and protocols.

Lead Quality Assurance & Performance Improvement (QAPI) initiatives.

Conduct physicals, reassessments, and develop treatment plans.

Participate in interdisciplinary team meetings & family conferences.

Educate patients and families on chronic conditions & care planning.

Collaborate across divisions and with community providers.

Ensure accurate clinical documentation & compliance.

Participate in provider on-call rotation as required.

Qualifications

Board Certified in Family Medicine or Internal Medicine โ€“ Required.

Board Certified in Geriatrics (Fellowship or CAQ) โ€“ Highly Preferred.

Active Indiana medical license (or ability to obtain).

CDS & DEA certification (by start date) โ€“ Required.

BLS certification โ€“ Required.

Minimum 1 year experience with frail/elderly population โ€“ Required.

Prior supervisory experience โ€“ Required.

No visa sponsorship (J1/H1B not supported).

Working Hours

Full-time Medical Director position

Standard weekday hours with occasional travel in-region

Participates in provider on-call rotation

Strong leadership, supervision & mentorship abilities.

Expertise in chronic disease management for older adults.

Excellent organizational, customer service & problem-solving skills.

Ability to thrive in a collaborative, interdisciplinary environment.

Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook).
