Family Medicine or Internal Medicine or Geriatric Medicine – Post Acute Care – Rochester, NY

Position Highlights:

  • Full-time or Part-time position in a skilled nursing facility setting
  • Flexible weekday rounding schedule (4–5 days/week)
  • Autonomy over daily schedule and patient care approach
  • Collaborate with interdisciplinary teams and facility leadership
  • Provide clinical oversight and mentorship to nurse practitioners

Job Duties:

  • Conduct primary care rounding for short- and long-term care patients
  • Diagnose and manage chronic and acute medical conditions
  • Develop and implement individualized care plans
  • Coordinate care with hospice, palliative, and home care providers
  • Participate in quality initiatives and case reviews
  • Engage in community-level relationship-building and network development

Compensation & Benefits:

  • Estimated annual compensation: $174,004 – $208,804 (productivity-based, no cap)

Candidate Requirements:

  • Minimum 1 year of primary care experience
  • Post-acute care experience preferred but not required
  • Active and unrestricted New York medical license (or in process)
  • DEA with prescriptive authority (or in process)
